I haven't confirmed with logs but the issue might be related to Preloader.

If so, Preloader just needs updated. I fixed the issue since I now maintain the extension so it could continue to work on Miraheze.

https://github.com/Universal-Omega/Preloader/commit/e0d0343c005f90c8936450eebea8fe0b89a1c5a2 should be the commit that fixes it. I'm unavailable to update right now but someone should just need to deploy https://github.com/miraheze/mediawiki/pull/4464

Does the issue still exist? It should be resolved as I deployed the update to Preloader.
