Create a bug report form on Phabricator and override the internal error exception message to mention it

Description

We should consider creating a new bug report form on Phabricator to make it easier for users to report bugs with more details given. We should also consider overriding the internalerror-fatal-exception message (I think?) with MirahezeMagic, to mention the new form with a link appended to the end of it (though we would probably have to override the whole message, just adding that), and make it easier and direct for how users can report bugs on Phabricator.

https://miraheze.wiki/br will now redirect to the bug report form, so that shorter URL can be used when overwriting the message, since wiki text or raw HTML URLs are not supported there. Only plain text,

I created the bug report form, but decided not to do message override for now, as there is no great way to mention the link in this message.
