I've temporarily closed it to prevent editing (just in case ™ ). Feel free to unclose it to do something to fix it. It seems like you (the DBA) have to fix it internally - and you have my full blessings to do that. Let's see if it happens again....

It would appear pages are creating in the main namespace (ID) and also in the user namespace.

While it is possible this may in fact be a valid issue, I am closing it as declined for the following reasons:

Since there is currently no active site notice about this, closing as resolved, next time we add a site notice, we can attempt to do this when initially adding it.

This used to be an issue, but I have not encountered it in a long time, I personally am able to submit site maps for my wiki to Google with no issue. I am assuming the issue is what Agent said above in regards to the status never changing.

Declined aa unmaintained and uses some outdated code. But it can be reconsidered if it becomes maintained, or if upstream patches can be done to fix outdated stuff.

I may submit an upstream patch for that.

This causes a flash of the screen on skinswiki so -1 and I expect a decline.

Thanks for proposing this. It makes sense that anonymous users have little reason to be concerned with these extensions, so we can exclude them.
